Granger Medical Summit Urology’s Patient Portal is a great way for patients to have easy and private access to medical information online (for the best viewing experience, it is recommended that you use Google Chrome to visit the Patient Portal website). On the Portal, you can view personal health records and receive periodic updates and reminders from your clinic.
To access the Patient Portal on a smartphone or other mobile device, download the healow app by eClinicalWorks LLC from the Apple App Store or Google Play Store. (Use practice code: HGCCCA when setting up your account on the app)

What To Bring
Prior to coming to one of our locations, keep in mind that it will expedite your visit/appointment if you bring the following items with you:
Referral (if required)
Your Driver’s License
Your insurance card
Co-payment/deductible (if required)
List of current prescription or over-the-counter medications (please include dosage/frequency)
Pertinent information about your medical and surgical history
Any related x-rays or records
Completed forms from this website or that our office may have sent
